Junior Player Forms

** Please go to "Registration and Sign Up" at left to register online with SignUpToPlay.com. The membership form will print out from Signup To Play with your information filled in (click on "My Info" at the top left of your Signup To Play homepage once logged in.) Be sure to follow the instructions for printing the form and make sure your pop-up blocker is disabled.

(Junior members, do NOT send these forms to the Badger Region office to pre-register for club tryouts. These forms should go to your club director and/or taken to each tryout, depending on individual club policy. Your club director will handle membership into the region. The region office will return these form to you, delaying your club tryout pre-registration.)
